Application
Below is the application needed to join West End Fire Company. Members are voted in during a meeting on the first Monday of every month. Prior to being voted in members must meet with an interviewing committee that will make sure the application has been filled out properly and all other paperwork is in order. This is also a time for you to ask any questions and for the committe to explain the roles and responsiblities of a member of West End Fire Company. We recommend that you hand in your application two weeks prior to the first Monday of the upcoming month. This will ensure the application proccess will be completed in a timely manner. Please print out the application and complete all sections, including the Physician Statement, Criminal Background Release, and the Junior Membership Waiver (if applicable). Follow the instructions below to access our application:

3. Click on the print button provided with the document on the
application site (Do not use the print button on your
browser!)
4. Fill out the application competely and drop off at West End
Fire Company two weeks prior to the first Monday of the
Month.
Criminal Background Check
West End Fire Company also requires all applicants to complete a Pennsylvania State Police Criminal Background Check. There is a $10.00 fee associated with this process that the applicant is responsible for. The link below gives you instant access to the PSP Crimianl Background Site and a completed background check form can be printed out instantly. Click on the link and fill out the required information. Once the background check is complete you have the option to print out the completed background check. Please make sure you follow all directions on the site competely and follow the prompts until the end before exiting out of your browser so your information is not lost. Once you have the PSP Criminal Background Check printed out, attach and submit it with your application packet.
